Official letter(s) from financial institution(s) on letterhead: applicant name, contact info, account numbers, dates opened, current & 6-month average balance, outstanding debts. Other supporting financial documents may be requested.
IMM 5409 (signed) + Evidence of 12+ months cohabitation: joint bank statements, lease agreements, utility bills showing shared address, other cohabitation evidence.
If divorced in Canada: divorce order or certificate issued by province/territory. If divorced outside Canada: legal certificate from that country/territory/region. Legal separation agreement (if available).
Adoption certificate + Legal adoption papers + Other relevant documents from the country/territory that formalized the adoption.
Both sides of family member's Canadian citizenship doc / birth certificate / PR card + Proof of current Canadian residence (lease, mortgage, utility bill -- within 6 months) + Proof of relationship (birth certificate, official document, passport inside back cover showing parents, relative's parents' marriage certificate, adoption documents, or other proof).
Letter of explanation regarding use of other name AND/OR legal documents of name change.
A notarized Custodianship Declaration (IMM 5646). The custodian must be a Canadian citizen or permanent resident, 19 years of age or older, residing in Canada. Note: Minor children between 17 and the age of majority (18 or 19 depending on province/territory) may need a custodian at an officer's discretion.
Court Order / Custody Decree confirming sole custody (issued by a court of competent jurisdiction; must clearly name the custodial parent and state that they have sole decision-making authority).
Letter of Authorization from the Non-Travelling Parent (signed; in English or French if possible) must include:
  • Non-travelling parent's full name
  • Non-travelling parent's address and telephone number
  • Photocopy of non-travelling parent's signed passport or national identity card attached to the letter
